FINANCIAL

Based on the above selection, I agree to pay the monthly amount for the care of this child. Payments are to be made monthly or annually. A late fee of $35.00 dollars shall be automatically added to all the payments received after the 3rd calendar day of the month. If an account becomes delinquent (over one week past due without receipt of payment) there will be an additional weekly service charge of five percent (5%) of the past due balance added automatically. In the event arrangements to make payment are not made, Milton Montessori School may, at its discretion, dismiss the child, and the parent shall remain responsible for the balance due, and any expenses incurred by Milton Montessori School in the pursuit of payment. Fees for each month is fixed and there will not be any discounts for the months that include spring/winter breaks or other holidays.
I agree to pay all the fees and tuition online using the school’s online payment system. 4% credit card processing fees will apply.
The families will pay a nonrefundable Application and Enrollment fee upon acceptance of the Application contract. The tuition may be paid on an annual or installment basis. (Discount of 1%, if paid full 10 months tuition at the beginning of the academic year. Paid in full tuition made with GASSO money is not eligible for any discount.
Tuition will be prorated for any student enrolling during the school year. Tuition will not be prorated during school absences. Tuition will be paid for each month of absence in order to reserve his/her seat. For a child to benefit from a 10- month program it is important to take vacation during the allotted holiday times. (See calendar on school website). Fees are calculated based on working days only, all holidays are removed before the calculation of all the fees.
I understand that full monthly tuition will be paid regardless of the number of days the school is open or the number of days the child is absent that month for ANY reason. NO REFUNDS WILL BE MADE FOR DAYS MISSED DUE TO ILLNESS, WEATHER, FAMILY EMERGENCIES OR HOLIDAYS.
A thirty day (30) written notice (withdrawal form) of your intention to withdraw your child is required by the school. Otherwise, you will be charged the following month's tuition. If you intend to leave the school, you will need to notify the school in writing on or before 30 days.
A late pick up fee of $2 for every minute will be applied if children are picked up after 6:00 pm and it is paid to the staff directly.

RECORDS AND MEDICAL

I agree to arrange for the necessary medical immunizations for my child prior to or within 30 days after enrollment and I will provide updated immunization reports as required thereafter; or I will submit the necessary documentation for medical or religious exemption from these requirements. I also agree to notify the school if any allergy information changes.
I agree to pick up or arrange to have my child picked up within the hour when notified that he or she develops symptoms of a communicable disease: a temperature of 100.4 or above or vomiting once or 2 incidents of diarrhea. My child will be able to return to school after a full school day has passed, with no symptom reducing medications.
I understand that in case of an emergency due to illness, the provider will contact the parents or guardian; if the parent(s) or guardian is not available or cannot be reached; the provider will notify the designated emergency contact to pick up the child.
I authorize Milton Montessori School to obtain immediate medical care, to provide or arrange for emergency transportation to the nearest emergency medical facility for my child, if an emergency occurs. I have completed, signed, and dated the child's emergency medical authorization form.
I understand that Milton Montessori School may only allow Epi Pens or Autoinjectors with a note from the doctor.
I understand that authorization for field trips may be given, and my child may be engaged in water sports or bicycle activities.

SCHOOL POLICY COMPLIANCE

I have received and read the MMS Parent Handbook. I agree to abide by the rules and regulations set by the school for health, safety and welfare of the child. I agree to abide by the policies and procedures outlined in the handbook.
MMS reserves the right to deny, cancel, sever or suspend a child's enrollment if deemed in the best interest of the school.
MMS will follow the discipline policy as outlined in the Milton Montessori School Handbook.
MMS follows Forsyth County inclement weather school closing policy, there will be NO TUITION REFUND FOR THE DAYS THE SCHOOL IS CLOSED BECAUSE OF INCLEMENT WEATHER. Please initial here
I agree to purchase the uniform for my child as specified in the Milton Montessori School Parent Handbook. I will bring my child to school wearing his/her uniform Monday thru Thursday, and on Fridays he/she will wear the Spirit wear attire.
Each academic year parents are required to update and submit the new Parental Agreement and if needed an updated emergency form for each child. Each family must submit per child the non- refundable registration fee for returning students Sibling discounts do not apply. 1. I understand that the school may use the photographs of my child in school albums, newsletters, websites, yearbooks and social media sites.

INFORMATION SHARING

I understand that Milton Montessori may sell videos of special events that may contain a child's image and or voice to school staff and families for fundraising activities.
I understand that Milton Montessori will print parents and child's names, address, emails, cellphones, etc in MMS directory and share with the MMS families upon request.
The Milton Montessori School considers the records of all students to be confidential information available to a child's parents upon request.
Records will only be released to other schools or agencies upon a written signed request from a parent or guardian.

LIABILITY

I agree to indemnify and hold harmless, and to absolve Milton Montessori School from liability for any injury, illness or accident which should befall my child or me on the school premises. Nothing contained in this paragraph shall be construed to absolve MMS from liability for consequences of their negligence acts or their willful misconduct.
